De oorkonde is uitgegeven op 27 mei 1944 en bevestigt de toekenning van het Verwundetenabzeichen in Schwarz (Zwart Verwondingsinsigne) nadat Karl Schmid op 14 mei 1944 nabij Kishinev gewond was geraakt. Het document is voorzien van de originele stempel van het Wehrmacht-veldhospitaal en de originele handtekening van de Oberstabsarzt (Chefarts).
